Years of new research shows that men need to watch out for their fertility. It was always known that male infertility depends on various internal and external factors that change naturally during life, but now experts warn that a series of factors can influence fertility of a healthy man. Process of creating new sperm, called spermatogenesis lasts from 75 – 85 days under the influence of a male sex hormone testosterone made by testicles.

Therefore in every cycle new sperm can differ from the last one, by the numbers and quality. And those elements are the important ones when we talk about male fertility. Sperm is made out of spermatozoa, secretion of sex glands and secretion of epididymis. Usually there are some cells included also like cells from the urinary tract, white blood cells or in some cases various microorganisms.

It’s totally normal that sperm changes from day to day even from a healthy male. To make an analysis of the sperm quality, men need to restrain from sex for 3-5 days to get more precise results. Semen  analysis is called spermiogram and its routine test to determine the fertility and presence of microorganisms and inflammatory cells in sex glands. Results can vary since spermiogram is not a perfect test and factors like smoking, alcohol, drugs or hormonal intakes along with individual oscillations like change of temperature inside the scrotum due to things like hot baths, saunas, elevated body temperature or some inflamminations, can affect the test. But the test can still show the overall reproduction function status of a male.

The most often cause of the male infertility is a smaller number of spermatozoa called oligospermia and weak mobility of spermatozoa called asthenospermia. Last studies show that today male infertility is the cause of more than 40% of infertility cases in marriage, while some 40 years ago it was around 20%. What is the exact cause of the increased male infertility is still under the debate, but researches show there is a progressive drop of overall number of sperm inside the seminal fluid, especially the mobile and quality spermatozoa.

Chaos in the system

The drop in the number of those mobile and quality spermatozoa in overall numbers is the main issue since those are the ones that can fertilize the egg cells. Even though that is not a final conclusion about risk of fertilization, since it takes only one in a millions, the continuation of this trend will raise that risk in the future. The research shows that larger numbers of men with smaller numbers of sperm also have weaker motility of spermatozoa (oligoasthenospermia), while men with weaker motility of spermatozoa still have the normal number of sperm in seminal fluid. In the special category are men that don’t have any motility in the sperm (necrospermia).

Normal sperm numbers consist of about 50% mobile spermatozoa and half of that of progressively motile spermatozoa – more important ones since they move forward. Most common causes of weak sperm motility are various acute and chronic infections in the urinary tract, bladder, prostate or testicles. In that case after the urology exam, it’s important to do microbiological tests like urine culture, semen culture, urethral swab for aerobic and anaerobic bacteria, chlamydia, mycoplasma and ureaplasma.

During the urology exam that includes the color doppler ultrasound of scrotum it’s easy to see possible problems like varicocele, a mass of varicose veins in the spermatic cord, since it can also be a reason for weak motility of sperm. Treatment for weak sperm motility depends on the cause, and the most frequent one is sexual infections that can be treated depending on antibiotic susceptibility testing (AST) by a specific antibiotic. If the cause is sexually transmitted diseases (STD) then both partners need to be treated to avoid returning of infection and making it more resistant to further treatments. Varicocele as a cause of male infertility can be treated with classic or laparoscopic operation where the veins are tied off.

Male powerful allies

L-arginine – is an amino acid that our body makes and is naturally found in foods containing protein. In studies it was found to be helpful in increasing sperm count.

Vitamins C and E – they help in sperm creation.

Tests showed that when people ate food with a high glycemic index, inflammation markers increased a lot in just a few hours after meal. Main characteristic of anemia is lower concentration of red blood cells and/or hemoglobin in one liter of blood. Hemoglobin is a protein that’s located inside of red blood cells and its role is to transfer oxygen from lungs to the cells in the body. Although anemia can occur due to lack of B12 vitamin, the most common cause is lack of iron in the body. This mineral is absorbed in duodenum and the beginning part of the small intestine and has a huge role in that absorption has gastric acid and vitamin C.

Skin, the largest human organ with size of around 2 square meters or 21 square feet acts like a first line of defense from harmful impacts of the environment. People can get infected by fungus through contact with other people or from various objects like clothes, shoes, hats, bed sheets, etc. Infection transfer is especially effective in large groups like kindergarten, schools and working environments. Also people can get infected by fungus from animals or fungi that are primarily living on the soil.

Humidly fertile surface

Healthy skin is excellent protection against various unwanted citizens, like fungus. But wet, damaged skin will serve as a great entry point for all kinds of microorganisms and help in their propagation and cause illness. Especially good places for that are ones where skin is under constant friction and thus easy to miss with naked eye.

For fungus to spread there has to be good conditions like humidity, temperature around 37 C or 99 F and skin pH acidity from 5 – 7. Some people are more prone to fungal infections due to various reasons like lower immune system, exhaustion and fatigue, long therapies with antibiotics and cytostatics, AIDS, etc.

For fungal diseases there are various names, but in practice there are three types – saprophytic fungus, dermatophyte fungus and systemic fungal infection.  Saprophytic fungus live and spread on the surface of the skin and they don’t cause any inflammations.  Dermatophyte fungus are destroying keratin and causing changes on skin, hair and nails. Systemic infections are the hardest fungal infection where infection is spreading to internal organs.

The most common fungal infection overall is saprophytic Pityriasis versicolor since they love humid and warm places. Therefore they are more prone to infect people that sweat more like athletes, physical workers, overweight people, etc. The infection doesn’t just cause an aesthetic problem but it has to be taken seriously due to part of skin affected by it becoming entry points for different infections. People need to arm themselves with patience since this type of fungal infection is very resilient to therapy. Treatments are done in short intervals over a longer period of time with various shampoos, spreyes and solutions that have antifungal effect.

Hygiene and persistence

If you have a problem with this type of fungal infection after showering you need to pay attention to remove skin peelings where most of the fungus reside. Also wear clothes made from natural fabrics and avoid synthetic clothes. Try to avoid all activities where you would sweat more and forget swimming in public pools for a while. Stay persistent with the therapy.

Dermatophyte fungus infections that cause inflammation on the skin are coming from a larger group of fungus like Microsporum canis, T. rubrum, T. mentagrophytes,  and sometimes yeast like Candida albicans. Yeasts are special group of fungus that can cause changes on mucosa, or cause some dangerous infection in organs.

Depending what is the cause of the infection, the clinical picture can be different. On smooth skin without hair there are usually pink-reddish spots that slowly spread and flake, but the center of the spot is recovering. On the scalp, except the skin flaking the hair itself starts to break so sometimes it looks like there isn’t any hair on those infected areas. It is very important to find the right cause. In other cases the fungus will spread deeper into skin and cause deep inflammation and pus nods.

Also other areas can be infected. If it’s nails they can become yellow with white spots that spread to the whole nail which eventually starts to crack, thickening or sometimes even fall out. If the infection is on the place where there is skin frictioned by movements, like areas between fingers, under the breasts, under the armpit or on thighs, than the skin becomes red, wet, flaking and away from infected area there starts to appear so called „satellite pustula“. On that spot there will be scratching sensation, and if you remove the skin flakes it shows a shiny light red surface.

Athlete’s foot

Changes can appear on hands and feet, mostly between the fingers and toes causing itching, red skin, cracks and sometimes bubbles. This type of infection often affects athletes and therefore comes the name „athlete’s foot“.

If you suspect a fungal infection, you should definitely consult with the dermatologist. After a series of specific tests the doctor will determine the proper diagnosis and according to that a proper way of treating it. Therapy depends on a number of factors with most important the area of infection (scalp, skin, nails), how deep infection is, inflammation and contagious. Main therapy includes antifungal drugs, but also there are creams, losions, sprays and shampoos which are used directly on the infected areas. Systemic therapy is needed when the infection is persistent and it spreads to bigger areas of skin. In case of bacterial superinfection there should be antibiotics included in therapy also.
